This is a fact that digital health tools which show promise during clinical trials often fail to do the same, when integrated with the real world, but a broader focus on patient engagement could help you ease that transition.

Clinical trials depend on serious efforts to make sure that patients are using a product correctly, in part as publishing standards need a low attrition rate among participants. Using social media, behavioural economics, and real world metrics will ensure patients and consumers get motivated to use the tools in long term.

Last year, the FDA released guidance that emphasizes the importance of real world evidences and real world data while evaluating novel medical devices.

Evidences are important means to convince healthcare executives to buy the products. But simply learning that an intervention was successful in a clinical trial is not sufficient. Real world validations are also important as digital health companies own the validation process from product development to scale.

Experts said the best patient engagement techniques mix low-tech and high-touch methods. Though service providers see the exact benefits of patient engagement tools, many are not sure which ones to recommend to patients. The fact is that about 50% of consumers are sceptical about healthcare technology that it adds another complicated layer to integration efforts.  

Many practices implemented a patient portal, but a big challenge yet remains and it is getting patients and doctors to actually use it.

Practices use portals for a variety of purposes. They can permit patients to schedule visits, bill payments, request prescription refills and communicate with physicians and staff. That convenience puts online portals at the top of a list of patient engagement strategies in a recent survey.

Portals could also increase efficiency by conferring some duties from doctors to their staff. That cuts down on some monotonous tasks, permits staff to focus on important work. But it can be really difficult to get patients to use the technology and for physicians to take advantage of all that a portal can offer.

You might desire to see 100% of the patients use it but you can’t imagine that could happen, but it would leave some hope; perhaps a few are doing it. Both physicians and patients could be blamed for the slow uptake of such technology, which is though relatively new for practices. ADHD Clinical Trials And It’s Various Approaches

Attention deficit hyperactivity disorder or ADHD affects about 1–3% of the children. There is a high level of co morbidity with developmental and learning problems and also a variety of psychiatric disorders. ADHD is highly heritable, although there is no single causal risk factor and non-inherited factors also contribute to its aetiology. The genetic and environmental risk factors that appear to be associated with a range of neurodevelopment and neuropsychiatric results, not just ADHD. The evidence till date suggests that both rare and multiple common genetic variants likely contribute to ADHD and modify its phenotype.

ADHD or a similar phenotype also appears to be more common in extreme low birth weight and premature children and those exposed to exceptional early adverse situations. In this review, the authors consider recent developments in the understanding of risk factors that influence ADHD.

While ADHD is well-studied in children, about 2.5-5% of the worldwide adult population also has ADHD and that population has a greater rate of binge eating disorders and dependence on alcohol.

That’s why clinical researchers in Sweden set out to examine whether this correlation is hereditary or environmental, and to carry out plans, they approached to the Swedish Twin Registry. Researchers examined 4 studies comprising more than 18,000 twin pairs aged between 20 and 45, which included both identical twins and fraternal twins.

In their study they found that environmental factors have very little to do with an ADHD patient’s chances of developing binge eating or alcohol dependence but common heredity problems for both disorders seemed responsible.

Building the Best Early-Stage ADHD Clinical Trials

ADHD research design for early stage clinical trials must agree with this correlation between dependency disorders, substance abuse behaviours and ADHD. When screening patients; mainly for small population of Phase 1 clinical trials; two things need to be considered—-their history of substance abuse and their family’s history of the same must be fully examined such that the most appropriate candidates could be selected for the trial. With all observations we know that an ADHD patient with a family history of alcohol dependency presents a very different case than that without.

Today, there is available only one pro drug stimulant: Lisdexamfetamine dimesylate (LDX, which is popularly known as Viviane by Shire U.S. Inc.), which is made for the treatment of ADHD in children aged 6 to 12 years. Therapeutically it is inactive before ingestion; LDX is converted into naturally-occurring l-lysine and active d-amphetamine after ingestion.

The diagnosis of ADHD is yet shrouded with controversies. While a few children do benefit from the intake of medicines, other children who behave differently than is the norm might be needlessly medicated.

This new report also points that a spotlight on a window of opportunity for neuroscience clinical trials: If ADHd can be treated early, then it can prevent dependency disorders from developing later in adulthood.
